As the parent/legal guardian of the minor child registered above, I hereby grant permission for my child to participate in all activities and outings organized by the Followers of Christ (FOC) program offered by St. George Coptic Orthodox Church. I acknowledge and agree that there are inherent risks associated with these activities. In consideration for allowing my child to participate, I hereby release, waive, and forever discharge St. George Coptic Orthodox Church, its employees, volunteers, agents, and FOC program organizers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by my child while participating in or traveling to/from these activities. This release is intended to be as broad and inclusive as permitted by law."
